Before a consult can be performed, a doctor / clinic / hospital account must be created. You can either fill out the form on line and electronically transmit it to us or you can download a form and fax it to us. Please note that if you fill out the form on line, someone will call your office for a credit card number to which to charge consultations.

Online Consult Form
Consultation requests received prior to 5 PM mountain time Monday through Friday will have replies by 6 AM mountain time the following day.
Routine consultations received after 5PM Friday will have replies by 6 AM Tuesday at the latest.
Requests which require immediate response must be phoned in.
